Preparation for Adulthood (PFA) Navigators Termly Report

Our latest PFA Navigators report reflects on the topics and themes raised with us over the term and summarises how PACC are supporting families by navigating the system options available to them and escalating issues where themes and trends are emerging or continuing.

This terms report highlights a continued trend of families with issues regarding EHCP delivery and support from the SEN Team, access to school provision (where children are on part-time time-tables or are at risk of placement breakdown), and the continued issue of practitioners sharing incorrect information woth families. We consistently raise these concerns through our escalation meetings as part of our Coproduction Partner roles and as part of the PFA Navigator responsibilities.
